YOUR DETAILS

THIS IS AN INTERACTIVE BLOCK. If you select the last option above, the rest of the survey will disappear leaving only the Submit button to press. (That might be the easy way out of this survey, but it would mean you won't get any match invitations! If you do this accidentally, close the page and re-open it.)

ON WHICH DAYS ARE YOU USUALLY AVAILABLE?

League matches are played on weekday evenings. County matches are played on Saturday afternoons.

TEAM CAPTAINCY

THIS IS AN INTERACTIVE BLOCK. If you answer by selecting the first option (willing to captain) a supplementary block will open below with more options.
Thank you for expressing your interest to help the Club in this way. Select those options which interest you. While we would expect any member interested in team captaincy to take on only one team, it would help when assigning captains to teams to have flexbility, hence this being a multiple choice question. (If you have selected the wrong option in the previous question, simply clear the ticks in this question, then go back and select "I am not available to captain any Club teams in 2023-24" to clear this supplementary.)

YOUR AVAILABILITY FOR MATCHES FOR THE CLUB IN 2023-24

Please indicate below your avilability for each League indicated.
